How Strong Is a Nicotine Pouch, Really?

The number printed on a can of nicotine pouches is one of the most misread figures in the whole category. Two cans can both say “20”, and one can contain twice the nicotine of the other. The reason is that manufacturers use two different units, and only one of them tells you your actual dose.

mg/g and mg per pouch are not the same number

  • mg per pouch is the dose. It is the total nicotine in the single pouch you are about to put in.
  • mg/g is a concentration. It tells you how much nicotine sits in each gram of the material, and says nothing about how much material there is.

The conversion is straightforward once you know the pouch weight:

mg per pouch = mg/g × pouch weight in grams

A pouch labelled 20 mg/g that weighs 0.7 g holds about 14 mg. The same 20 mg/g in a slim 0.5 g format holds 10 mg. Same headline number, 40 percent less nicotine.

This is where the famously “extreme” brands get their reputation. A can advertised as 50 mg is usually 50 mg/g, and at a typical 0.6 g pouch that works out to roughly 30 mg per pouch. Still very strong, but not the number on the front.

Typical strength ranges by brand

Ranges differ by market and change with product revisions, so treat these as orientation rather than a spec sheet, and check the can you actually hold:

Two pouches at the same mg per pouch can still feel different. Moisture, pH and pouch size all affect how quickly the nicotine is released, which is why a moist 10 mg pouch often hits harder in the first few minutes than a dry one of identical strength.

How much of it actually reaches you

The number on the can is content, not delivery. During normal use, on the order of a third of the nicotine in a pouch crosses into your bloodstream. The rest stays in the pouch or is swallowed and largely broken down by the liver before it does anything.

The comparison to cigarettes is more interesting than most people expect. A 2025 systematic review and meta-analysis in Drug and Alcohol Dependence Reports pooled seven controlled trials and found:

  • Total nicotine exposure from a 4 mg pouch reached 91.7 percent (95% CI 85.0 to 98.4) of a cigarette’s.
  • Peak blood concentration was clearly lower, around 69 percent of a cigarette’s.
  • Time to that peak was 20 to 65 minutes for pouches versus 5 to 8 minutes for cigarettes.

So a pouch is not a milder dose overall. It is a similar total dose delivered slowly. That is why a pouch feels less like a hit and more like a plateau, and also why the number of pouches per day matters far more for your total intake than the strength printed on any single can.

What this means in practice

If you are choosing a first strength, start at the bottom of the range, 3 to 6 mg per pouch. Tolerance climbs quickly and is uncomfortable to reverse, so the cheapest decision you will ever make here is the first one.

If you are trying to cut down, lowering the strength alone rarely works. People compensate by using more pouches, which is why total daily nicotine often stays flat while the can looks weaker. Counting pouches per day is the honest measure, and lowering that number is the change that shows up.

If you are comparing brands, convert everything to mg per pouch before you compare anything. Otherwise you are comparing a concentration to a dose.

Strength on the can is fixed. How many you get through in a day is not, and that is the figure that decides your intake, your spending and how hard quitting will be later.
